Basically, any "Open file" dialog in any program is completely unusable and simply selects a random folder to display, with no way to select another one.

I've got a custom VS installed, I've tried reverting. Doesn't help.
I've no idea what a VS is. I'm assuming it's either a damaged file in Windows or some application is breaking it. Open command prompt and type "sfc /scannow" without quotation marks to check and fix damaged Windows files
Visual style. It involves patching a few files, manually adding theme files into C:\Windows\Resources\Themes, and adding a modified ExplorerFrame.dll to System32. I'm not sure if doing the scannow would undo this.
Oh, well you've basically just diagnosed your own problem. You will have to get the unpatched files back
