NS1 had a ton of maps with non-solid world faces where players could enter. A variety of things caused them, with the usual culprits being malformed world geometry, malformed portals or improperly built levels. Mind you, they weren't caused by buggy game code. Some examples: ns_metal (parts of the center column in Surface Access Hive disappear at certain angles.) ns_lucid (The rock geometry is really buggy in the Void Hive and goes null/player can fall through in certain spots.) co_faceoff (players can fall inside the double stacked red crates in the central room if you wedge yourself between the top one and the wall.) ns_siege2k4_rc8 - has the largest void I've ever seen, which is basically a huge triangular column that extends to the top of the world (+4096). You can fall into it by pushing against the back wall on the catwalk in the room just outside of marine start.
