Perhaps it is tested for HIV only once it arrives in the hospital, leading to a mix-up in the stores of clean and infected blood.
I'm a donator myself and I can tell that even donator blood isn't entirely safe. They test it on things like HIV but they can't test it on everything. That's why they rely on people to be as honest as possible when donating blood, that's also the reason why you don't get paid for that in the Netherlands.

I know the chance is super slim, but this is one of my nightmares every time I go in to a surgery. Usually even more than the risks the surgery brings.

You're right, but they are (ironically to a degree) similar to the morning after pill - you need to take them very quickly after exposure or possible exposure to HIV, and I'm not sure if it works if the infection is transmitted via blood.
Also, letting known HIV positive blood into the transfusion chain is a massive, massive lapse in biosecurity - I'm not sure how you could fuck up that badly...

Gay people aren't allowed to give blood here because they feel the chance of HIV infected blood making it through is too high. They can't test all the blood, there's not enough time.

No, all blood is tested for several diseases. Gay men, sex workers and drug users were all previously denied the ability to give blood - rightfully, gay men now have the ability to give blood, as long it's within 12 months after having sex with another man.
The twelve month period is because a) sadly, gay men currently suffer from a disproportionately high level of HIV and b) HIV has a window period after infection where it is impossible to detect, but still transmissible.
There is still a very small chance of transmission if someone who is HIV donates blood and it isn't picked up, but if someone is HIV positive they shouldn't be donating blood anyway.
