Hilarious reactions aside, how the hell did he do that? That's one of the few tricks I've seen him do that genuinely fooled me.

As far as the no blood thing I know you have a little bit of time before the blood begins to flow. When I was a kid I was building a ramp with my brother with some old wood we found near our house. While we were putting it together I put my hand under the ramp somewhere and my brother hit the other end of the ramp which caused an old nail to sail through my hand. My immediate reaction made me pull my hand back and with that I pulled the nail out. I was surprised by two things, first there wasn't much pain (although till this day that accident caused nerve damage in my hand) and there was almost no blood flowing out of it for around 30 seconds. With that knowledge I really wouldn't be surprised if he really did stick that pick through his hand.

I dunno. Pretty sure he is just enduring the pain, which is such a clever move, when you're as dedicated as he is.
He probably figured out the least painful/damaging spot. Maybe applied some numbing substance beforehand.

He seems to treat doing it as just doing an illusion- as if it was just a clever trick. That may be part of ignoring the pain.
He actually seemed like we was pretty concerned when Kayne was reluctant to pull it out. He just kept saying "Pull it out, pull it out" faster and faster.

Probably tissue damage but not a nasty infection. I'm guessing he cleaned the pick :v:

It's like ear piercing or gauging. Over a long period of time shoving the needle deeper and deeper, you'll pretty much have a hole hidden by scar tissue.
The hand remains very motionless through the action, and in the ricky gervais video the shoulder is all messed up. I'm thinking its a highly realistic prosthetic sleeve/glove and maybe part of the ribs. He could slide his hand out without it looking weird if there is enough support, and in both videos he positions the arm very precisely. He can just put his arm back in after the needle is taken out to move it again. Also watch the finger pop with plastic sound in 2nd video around 4:20. There is nothing here, he is just sticking a pick through his hand. There is no sleight of hand tricks or complex angles. I respect David Blaine for this, some might think this is macabre but I find it fascinating that he has so much dedication to his craft that he will dedicate 13 years (like it says in the article) to a single trick.
