Just watching it now, its not like he knew she was a cancer patient. It's not like she goes "I have cancer, sign this autograph" that'd just be a bit... odd.
Still would come off as awfully dickish, though.

If you were a superstar and had people asking you for an autograph every time you left your house you'd probably stop looking at them. How is he going to pick out one cancer patient out of a crowd of hundreds of people?
Also, lots of people wear pink; not just cancer patients. Who the hell is going to notice a tiny ass pin they're wearing? As I said, he probably doesn't look at people that closely. Third, bald? Lots of guys are bald, she probably kind of looked like a guy to anyone not paying attention.
