Alfred is a mentally ill dog who has schizophrenia which derives from his past experience when he was a child. His parent's abandoned him on his (first) birthday and was subject to molestation and sexual abuse. To find comfort away from reality, his mind created a 'Playhouse' where he does whatever he wants and the playhouse 'loves' him since no one else does.
