It only happened because they didn't use British Scaffolding, not Ladders with wood panels on them as I can see in that picture.

I think I once heard a theory that it isn't the actual fall that kills you, but the shock/realization (?) when you're a split second away from the ground. Sounds reasonable, being literally scared to death.
