Doctor trained to go entirely off of the heart rate. Heart for whatever reason slows to where pulse is undetectable, but the motor's still running. I've read stories of people ending up in a morgue freezer and waking up only to freeze to death. There was a period where they would install bells or horns leading down to the casket for graves because of the fear of this. Number one horrible fear? Being embalmed alive.

That's why they used to tie little bells down to the coffin and up to the tombstone in case someone came back to life and alert the groundskeeper.
