I can't imagine how hard this is to make. A lot of precision is required.

If you do it without any tricks, yes. Otherwise I'd take a picture of the ground, edit the picture on a PC, project it from the same position and angle onto the ground, then draw it.

That is pretty fast done yes, but they painted it on the ground, that is about 90x harder

Illusion or not, that's still probably scare the fuck out of me.
It's a shame that the way it was made made the 'falling off' slab look like it was bending downwards, so it looked like the guy jumped about 3 metres, as well as progressively getting larger as he walked down it.
It isn't painted.
