I'm not sure if you're asking for the most controversial game, or the most revolutionary game. Your post seems to describe the latter, so I'll go with that. I think I'll go with Mario, without it most platformers wouldn't be what they are today.

Most controversial: Manhunt or Dante's Inferno. The first for having over-the-top gruesome death scenes, like de-nutting a person with a sickle. The second for depicting violence towards infants. The most history changing, making, influencing, unique at its time game ever: The first Mario. It's hardly arguable. It revolutionized video gaming.
I'd say Postal 2 is pretty damn controversial, but not as well known. Throwing anthrax at people, burning cats and dogs, many, many, MANY racial stereotypes, etc. Most history changing is Mario, no duh.
