A man is dead after a dual wheel fell off a tractor-trailer on a highway in Ottawa early Friday morning, then struck a cargo van with enough force to peel back the van's roof. A tractor-trailer was heading west just before 6 a.m. ET on Highway 417 near Carling Avenue when the driver noticed smoke coming from the wheels and began to slow down, according to OPP Const. Eric Booth. "As he was pulling over, a set of wheels from the trailer comes unattached, bounces across three lanes of roadway, goes over the centre median and then lands right in the windshield of a van as it's travelling eastbound in the far right-hand lane," Booth said. The force of the crash tore most of the cargo van's roof off, and the driver was killed instantly, Booth said.
Source: http://www.cbc.ca/news/canada/ottawa/fatal-crash-highway-417-1.3975572 This is what happens when a 150 pound object travelling at highway speeds collides with something travelling in the opposite direction at highway speeds, not a pretty sight.
Take that highway every day, that's a shitty way to die. Driver was arrested too.
That's some Final Destination shit right there. Apparently it was negligence. This could've been avoided.
Objects flying into cars at highway speed are never a pretty thing. My cousin had a guardrail section embedded into the passenger compartment of her PT Cruiser, luckily she was alone, otherwise: https://www.youtube.com/watch?v=iazTQVi1CEE (edit: not visually gory, but not safe for life; view at own risk)

Jesus, I can't imagine the sight of what was left of the poor bastard. That's why I always triple check the nuts when I change my tires.

It says graphic in the video title to be fair.
That's some Final Destination shit right there. Apparently it was negligence. This could've been avoided.
It never said it was negligence, it did however say that he had done his pre trip inspection that morning. If he done his inspection and the tires had sufficient tread (2/32 inch tread depth on all tires but steering) then I do not see how he was negligent. It says he was arrested and charged with operating an unsafe vehicle, probably a formality since something mechanical failed and caused death and until they find out whether or not he did his pre trip properly people should not speculate it's his fault if on his pre trip everything was within safe standards he shouldn't be charged, but just because a truck passes minimums on the inspection doesn't mean something will not break. He was also charged with driving with a detached part? are you serious? The wheel fell off during transit and he immediately pulled over it's not like he took it off and said "hmm I think I'm going to drive without my wheel today." Now I am not saying that if he is found to have been negligent by lying on the pre trip or just being too lazy to do it properly that he shouldn't be arrested, but the general population usually goes on a witch hunt when it comes to accidents involving commercial drivers and they just automatically assume that because someone died or an injury occured, which is fairly common considering they have GCVW of up to 80Klbs, that is is the truck drivers fault.
My dad is a supervisor at UPS, and one night he came home and told me that that night, someone driving the opposite way on the highway threw a frozen water bottle at one of his drivers coming the other way. Guy wasn't hurt but that could've fucking killed him so easy

Could've gone my whole life without seeing and listening to that video tbh. For god's sake don't embed that sort of thing, and put a proper warning. Now back to your regularly scheduled posting.

Unless it was clearly negligence, what is the point? It's not like they don't know who he is.
OPP runs roadside checks on tractor trailers every few years and each time the number of trucks pulled for either an unsafe vehicle/trailer or fake tags is kinda worrying. The number a couple years back indicated that of 200 trucks inspected at random on one roadside check 40 were impounded until repairs could be made, 12 more were told they would be permanently off the road and another three were found to have outstanding traffic and parking violations in multiple provinces. I don't remember the number of drivers fined for unsafe loads. Out here the coming of spring comes with the discovery of hundreds of "piss bottles" as truckers piss in whatever is in the cab and toss it out the window. The number of inspections aren't so much the problem as the penalties are too lenient. Moreso when you get trucks in accident that you find are already on CVSE's radar for failing a roadside check.

They also eat, drink, smoke and do all kinds of other crazy shit from behind the wheel. I remember sitting next to a guy who had one hand on the wheel, another on his phone as he argued with his bank about a credit card. Stick a guy in the drivers seat of a truck for 8 hours a day, 6 days a week and they're not going to give a shit.
Driver was arrested too.
Why?

It should also help fix the issue with trucking companies making unrealistic deadlines.
