One wonders why russia and china are so content to have a nuclear armed madman next door, moscow is a much easier target than washington dc after all. The communist party of china at least seems to feel the tension, people put up with a lot because they promise security and a nuke going off next door isn't much in the way of security especially since their wealth is intrinsically tied to the global economy now, its undercutting Xi's image. Russia obviously has much less exposure to this and aiding them gets them assurances the missiles only fly east i suppose

I imagine a lot of it has to do with propping up a state you don't want to see collapse. If the NK regime collapses, Russia is going to see a ton of spill over from those fleeing the North, and this is something they don't want to deal with. But of course the main reason is geopolitical. If the NK regime collapses, SK and the US are going to step in and install a friendly government.

I think your second suggestion, that it is about ideology, and historical tensions, is correct.

Makes sense. Prop up the regime, cause war between NK and the US, which further destabilizes the West. Allows Russia to increase their influence even more over the West.
