Unfortunately accident happen, especially when dealing with demilitarised, which is to say old, outdated and potentially dangerous, ordnance.

This is why old ordinance should be disposed of properly and not used in training excersizes. 60mm mortars are some pretty powerful ordinance, they should be thankful that the casualties were not any higher.
