Emergency services received a call about a woman lying motionless in the home 31 hours before she was found dead with her distraught toddler by her side.
Ambulance Victoria visited the apartment shortly before 2pm on Monday after receiving a call from a neighbour, but left after finding the curtains drawn and no one answering calls.
Victoria Police was also asked to conduct a welfare check to the Tongue St unit in Yarraville on the same day but left when no one could be raised.
It was not until a concerned resident contacted an off-duty police officer, who sent local police to the address at 9pm on Tuesday, that the body of Jemma Burton, 25, was found.
Ms Burton, also known as Jemima, was discovered with her severely dehydrated toddler, Toby, by her side.
It is not known how long Toby, who had just learnt to walk, had been trapped without any food or drink.
Police say the 15-month-old just wanted to be held by officers who attended the tragic scene.

When picked up by officers, he was reported to be clingy and didn't want to let go.
He was rushed to the Royal Children's Hospital where he underwent immediate fluid replacement and treatment for severe nappy rash.
He was released yesterday in a satisfactory condition.
It is believed the infant's father lives in Hobart.
Victoria Police confirmed it was asked to conduct a welfare check at 3.20pm on Monday.
"There was no answer at the door and at that stage police had no power to enter the premises so they left," Victoria Police spokeswoman Leonie Johnson said.
The Homicide Squad and forensic police initially investigated the tragic scene but yesterday confirmed they were no longer treating the death as suspicious.
Ambulance Victoria have defended the actions of the paramedics.
"They spent considerable time knocking on the door and windows to establish whether there was a patient, however, there was no response," acting group manager Greg Gibson told 3AW today.
"Given the conflicting description of the scene, and the lack of evidence suggesting that anyone was inside, the crew then cleared the scene.
"The person who made the emergency call was contacted again to give more detail but couldn't be specific and paramedics left after an considerable external search of the flat.
"On the call back the person was quite vague. Basically they said, look, dont worry about it."
Police and ambulance officials have vowed to conduct a thorough investigation of the incident.

The first thing I thought about when I saw her picture were "Drugs", I don't know if that's actually what did her in though.
she's looks so young too. I hope it was an accident and not anything drug related, please let her have been responsible and this just a horrible accident.
