The screenshots come from an actual shower scene in the game—but the scene never shows Page's bare breasts:

Not surprising, the artist probably figured there was no point in chopping off bits post-design, and i guess all you'd need to do to show bits would be find the 2 vectors that control the camera's position and angle.

There's full frontal cock in one of the GTAIV episodes, and none of them got an Ao rating, so it would've probably slipped past with an M.

David Cage is kind of a creep actually. He seems to insist on every game he does having a scene where the main female character of the game is either walking around home wearing as little as possible or just having a shower scene that never contributes ANYTHING to the story or anything.

Most video game characters are modeled nude to begin with these days. This makes modeling clothing on them much easier as you have an underlying anatomy to work with so it doesn't look unnatural. It also makes it much easier to make different sets of clothing.

Most of the time this model is just left as is if it even makes it into the game, so even if they use it in a scene where it's creatively censored, it'll still be the fully detailed model.

They did this on Max Payne 2. There's a shower scene featuring Mona Sax where you can't actually see anything, but with a little tweaking there's a way to replace Max Payne with the nude Mona Sax model and it's mostly fully detailed. It's much more common these days though, as more character artists start with a nude base model.

Thus I kind of doubt they had planned to show everything originally. The character artist probably just modeled her nude first as a part of his or her workflow, and they used whatever they had on hand when the concept of a shower scene came up.

One explanation I've heard (for why one of the FF13 characters had nipples modeled even though they were never actually shown in-game) is that they're used for making sure the camera angles used don't show parts that logically *should* be nipples.

That explanation made sense for that game, as the nipples were horribly low-res, just a darker splotch really. Here, they're modeled at full quality. So either some modeler had a bit of fun, or they actually were intended at some point.
