The paintball guns are really silly and wasteful. I remember there was some vest that you could wear to simulate you being hit by bullets and stuff, what ever happened to that?

That gives the player that adrenaline and fear fueled drive.
Midgets should rush out of the walls and beat your shins with bats when you take fall damage.
Now connect all this to a manually controlled surrogate cybernetic soldier. Done

I find that the biggest thing that can take you out of the game is a lack of fear. Gamers have nothing to truly fear when playing a game as its all just simulation. Nothing to really lose physically. That leads them to make decisions they would never make in a real scenario [like running into the open and gunning]. Put that fear of pain into a gamer, and he will have to stop and think.
