This is actually very interesting, something that's been mentioned on QI is that they let him play the guitar
to see if they aren't tampering with the wrong parts of the brain.
Something like real time feedback so to say.

iirc you have to be awake for it. They want to make sure they don't fuck up.
Also once they're past your skin there's no pain receptors in the brain itself.

It's like, you know you can do something, and you love doing it, but your body just refuses to do it. It just doesn't work. You can try again, and again, and again, and again, and every single time, no matter how close you get to doing it right, your hands will just move themselves at the last moment and fuck it up.

Correct me if I'm wrong, but generally anesthetic can't be used on most cranial surgeries (After popping open the skull cap). Thus if you're going to be awake, you might as well doing something calming.
