There's something absolutely terrifying to me about having brain surgery while awake. Maybe I shouldn't have watched that Ray Liotta scene from Hannibal
This is actually very interesting, something that's been mentioned on QI is that they let him play the guitar to see if they aren't tampering with the wrong parts of the brain. Something like real time feedback so to say.

Also once they're past your skin there's no pain receptors in the brain itself.

It just doesn't work. You can try again, and again, and again, and again, and every single time, no matter how close you get to doing it right, your hands will just move themselves at the last moment and fuck it up.
I'd like to imagine the doctor could tinker with different parts of his brain and make him play other songs.

Correct me if I'm wrong, but generally anesthetic can't be used on most cranial surgeries (After popping open the skull cap). Thus if you're going to be awake, you might as well doing something calming.
