I think it's partially due to fit in with the Sao paula(sp?) people. He looks.. stupid, but eh. Whatcha gonna do.

He shaves his head at some point in the game though, he doesn't start like this.

It's justified within the story, he shaves his head to reassert control over himself and his own life after dealing with his addiction. It's actually quite common for people to do this in one way or another after going through traumatic experiences.

according to the first trailer, he has regular hair and an unshaved beard in the flashbacks, starts to have longer hair/beard when he arrives at Sao Paulo, then some shit goes wrong and he shaves his head but keeps the beard.
