Yes they notice. They see themselves getting fat and they say "I'll eat less next meal." but it just tastes too god damn good to stop.

Of course there is also the chance of mental problems (most common) and actual physical disorder (most common excuse/least common cause).
I don't really feel like getting into a discussion on the complexities of eating disorders as I believe I may have urinary tract problem that is quite painful. Just throwin' that out there.
TL;DR
Some people actually try to get fat, most often stemming from sexual assualt of some kind, and believing this will prevent it from happening again. (due to less physical attractiveness)

I understand completely where you are coming from. After what happened to me, I colored my blonde hair to dark brown, hoping it would make me not so noticable. I stopped fixing it the cute styles that I did before. I stopped exercising, because I wanted my body to not be attractive to anyone else. I stopped wearing shorts and tank tops, I still do not wear them as much as I use to. I know for me, it is a saftey issue, I feel if I hide myself, and not appear attractive then guys will not notice me, then nothing will happen to me again. I know this is a silly way of thinking, but it helps me to stay comfortable in public. I just now started working out again. I just now feel comfortable enough to go apply for a part time job, but it is not working with any men. I do not think I will ever feel comfortable enough to work with men in a small setting. BUT I do know that it is not healthy for me to gain weight and hid beneath fear...I am working on it. Take your time and know that your are not alone.

The reason one of my friends from primary school is fat is because:
His mum wanted him to eat more because he was too skinny.
But the kid kept on eating, and then became fat.
Never tell your kid to eat more.

I didn't really care about the fact that I got fatter as a kid because my parents or anyone else didn't seem to care. I only started caring about it nearer to my seventh grade when we started jogging few times a week with my dad, that's when my weight gain stopped for some time and the losing began.
Now I've lost probably 15-18kg from those days and go running and to the gym frequently. I'm in better shape than ever in the last 5 or so years.
I don't know what I'd look like right now if we hadn't started excercising back then, would have probably gained even more weight and become obese.
