It's not every time, but once every few times I sneeze I get this really bad pain in my dick. It feels like someone is trying to push a needle out of my cock through the urethra. There's always the possibility you have a musculature issue stemming from an 'out' back. I have a muscle that spans around my thigh to my groin that originates from my 4th lumbar something-something that occasionally gets 'agitated' and causes me pain. If I sneeze while it's inflamed, it can cause pain throughout my groin, including my poor schnitzel. Just a thought.
