The doctor explains at about 14:00 that hygiene has nothing to do with the formation of those blackheads.
It looks like it gets really bad for a lot of them. Hygiene does play a part, if you wash your face with water like the doctor says in the beginning that should help too. God I would have so much fun popping every single one of those spots with my bare hands and safety goggles.
