He hits a guard once, he needs to hit him with 2 arrows to kill him. He kept missing the second shot. I'm guessing he looses his chance if he wont kill that guard fast.

It's 3 guys, but the third guy runs away by the looks of it, giving you only enough time to fire one arrow.

I'm just going to assume on my current playthrough that Roggvir was spared the block, given the first time I entered Solitude was while on the frontline of the Stormcloak invasion force taking the city...

Not on mine. He was apparently executed in my save file when the first time I entered was part of the Stormcloak invasion as well.
He's an awful assassin.

I've found like 3 of them in other dungeon chests, you can't only get it from him.
I've found plenty of amulets of talos. I stopped using it when I got the Gauldur Amulet
