When he pops up right behind you it makes you jump. Otherwise not really scary.
It's subconscious horror. Ever had a nightmare where you've suffered an inability to walk or run?
Or you were being stalked / followed?
Combined with ambient and the natural fear of darkness (or; the "unknown"), that's what makes Slender "Scary". Some people are more prone to it than others, though.

Funny enough Slender is completly based on shock scares.

Even the jump scare isn't that scary. I made my friend play Slender, and I yelled at him to make him jump, and he actually found that more frightening than the game itself.
