This happened to me as well. I found him in the woods earlier on in the game when I was going to the Graybeards. He was my first companion at the time and I would usually go into caves with him. Then one time I went into this huge cave with these vampires and forgot to save. He got surrounded by them and they drained the life from him. I only noticed he was dead after I killed them all. I burst into tears just by the sight of him lying there. I just said 'fuck it' and loaded back to when I was just inside the cave. I told him to go home, and watched him run to the horizon...into a Giant that turned him into a fucking spaceship.
Then I ate tuna.
Meeko helped me kill 3 dragons then a bear just randomly mauled him.
I killed the bear, skinned it, torched it, then i ate some of it, and dropped it in the river.
