It's been implied that the Adviser you see in the Citadel is Doctor Breen in a host body, due to the model being named Adviser_Breen and it attacking Alyx and Gordon unprovoked.

It's a bit of a stretch, but I suppose it's impossible now since, y'know, his voice actor is dead and stuff.
