A close relative of mine gets methadone at the local drugstore. She was a heroin addict a couple decades ago.
How does this stuff work? How long must they take it? Is it safe?

Sounds like they use it to ween addicts off of opiate based addictions. So it's not gonna be safe in either lower doses as well as high doses. It's just a weaker base of heroin pretty much.

It's a substitute for Heroin more often than not, however it is almost as addictive but not as destructive afaik. If she's still taking it a couple decades later she's pretty much addicted.
I've taken it for the buzz before, takes a long time to come on but lasts a while.
Methadone is considerably more difficult to come off than Heroin. I disagree with it being used as an analgesic and opiate supplement as it has factored in so many more deaths than heroin has in the last year.

But if someone knew methadone is stronger, what's really STRONGER than methadone?

Carfentanil actually isn't the strongest opiate, this actually is: [url]http://www.opioids.com/ohmefentanyl/structure.html[/url]
With a potency of 18,000 fold of morphine. Article says there's a good chance that that can even be made more powerful than it already is now.
I don't know of anywhere where this is actually used though, but yeah, Fentanyl, Carfentanil and Etorphine are more powerful opiates than methadone.
As far as practical opiates that are more potent, they use oxymorphone, and hydromorphone under the brand names opana and dilaudid respectively
Wasn't certain that Oxymorphone or Hydromorphone were actually stronger than Methadone, but they indeed are.
They're second-line from Fentanyl for reference.
Methadone is a synthetic opioid used to ween addicts off of Heroin, by giving them progressively smaller dosages, and the quality can always be assured.
Fentanyl is the first that comes to mind when I think "worst (best?) opiate".
And Meth is the first that comes to mind when I think "worst drug".
I'd even put Coke above heroin, but that's just me. :shrug:

I think it is based on the possibility of OD too. Heroin is notorious for causing deaths, addies are not.
