The sickness can only be the Necromorph infestation, but at the same time it ties in with Unitology beliefs that the Necromorphs are holy beings, in that they will lead Unitologists to enlightenment by altering their body and releasing their soul.

The Marker is still affecting Clarke's mind, so that's why he's apparently gone mad. Hopefully, this madness doesn't turn into full-on Unitological beliefs during the game, especially in the event that Isaac does speak in this game beyond angered grunts.

The Necro's must've followed Clarke or something because of the Marker's imprint. The Black Marker may also appear, so maybe that could be used to contain the infection before the entire station dies.
