Aside from the shitty and hilarious final level RSV2 is actually pretty good on this topic
On realistic difficulty if you get shot at for more than a second and a half 99% of the time you're dead.
In fact this made the game at that point extremely difficult and the only real time regenerative health ever occurs is in extremely lucky situations where you've just pulled back into cover after shooting.

I don't think I ever would have beat the game on the hardest difficulty if not for playing co-op. That helicopter is a royal pain in the dick.
