Well it's not outright giving a handjob but it's stimulating a spot that they enjoy/react positively too, it seems to vary a lot from cat to cat. My cats just pushes their ass up in the air and seem to enjoy it but they don't make any noises or anythimg.

My cat's lower back near her tail was very sensitive, but it seemed painful to her rather than pleasurable.
She reacted similarly but her back there always had an eaten up/infected look to it and she freaked out when anything (corner of a box, anything she sat down by that rubbed her fur, etc) touched it

Even worse for me my dogs male.
I always knew it had something to do with sexual excitement or reproduction when you petted a cat above its tail, causing the reaction.
And I first noticed this bodily function of a cat when I was way younger, no significant education or anything, I never even had my own cat, but still it was quite clear what's going on when you petted a cat above its tail..
After all, we all have those sweet-spots, so we can all relate to the feeling.
