Cartridge tilting originally came from the issues that occurred when you partially inserted the cartridge which would, if done at the right angle, cause artifacts in the game (or it would crash the game aka 95% of the time). Sometimes people use it more generally to describe when the flow of data is interrupted in some way from the cartridge to the game console.
He probably screwed with the prongs or an IC in the card itself to cause this time happen since multiple parts of the game are screwed up that would almost be impossible by just tilting the cartridge.

Definitely not cartridge tilting. It's not apparent to me why this is happening - I know it's probably not modified, so it is most likely a cartridge that somehow got corrupted. It could also be an issue with the console itself.

Yes, it seems to be the console. Corrupted data or improper connection would almost certainly cause the system to halt, so it really doesn't seem likely at all that it's the cartridge. Instead, there are tons of glitches with textures and some audio problems which points to faulty logic processing circuitry in the console.
The only other possibility is that there's some form of dynamically-loaded rendering code run from the cartridge at runtime, but that wouldn't explain errors in the audio.
If anything, I'd bet that other games have similar behavior on that particular system.

I found it to be, more of annoying, than actually scary.
I was expecting like creepy pasta scary, but it's literally just a fuck load of graphics glitches and sound glitches, nothing that great.

This isn't creepy in the least. I've seen this numerous times already anyway. It can be done with cart tilting or gameshark codes.
