It seems like he was trying to change lanes or some kind of dumb shit on the turn which made him lose control and swerve left really hard.

Its simple. The can ran into a semi which then turned away to get away from the car but didn't think about the median (or lost control) and since his semi is extremely heavy it's velocity made it go over the median.
