Used to have that problem back in gmod a couple of years back, turned out it was my gpu failing me. Then my card died.

Also check for some overheating. I had my CPU overheating few days ago like crazy (temperature jumped up to 100 C' ... had to put new thermal paste). It was similar to yours, running ok till it overheats then slowly loses temperature while FPS were down by like 40-100 in most of the games. So, just check your temp, might also reconsider deleting your gpu drivers -> restarting the PC -> installing new one. Check other (older) drivers on your GPU if that happens there too.

Try cleaning the graphics card. Maybe there are some dust in there that makes it run slower some times.

Yeah, that clearly looks like overheating. Post your temps before you try anything btw. Wait, GTA IV doesn't lag? Nor other games? Hmm...maybe it ain't overheating, but post your temps anyways.

You can download a few programs that give you the internal diagnostics of your hardware; temperature etc. I will have a look around unless someone knows one of the bat.

Try updating/re-installing the drivers since I have no idea what else could be causing that.
