Exaggerated example: If you were told by your parents you were the opposite sex when you were born but your parents changed it, do you think you'd feel worried?
He's worried that he has lived up (since the stroke) to a lie. It's not really an issue, it's just something most people don't feel comfortable about.

The brain controls everything about you as a consciousness - It is improbable but certainly not impossible for a stroke to cause a major personality change.
I liked what the stroke-doctor guy said. It makes sense, you are born with a pre-configured personality and you alter that slowly throughout your life. A stroke can kill neurons, so they have to rewire themselves to stay connected. And when they do, your personality changes, but in a more random and drastic way because your brain's priority is to survive and not to keep your personality.
If this is true, you could in theory be both born and socially constructed gay. But of course, if you're born straight it's harder to become gay through social construction, since you normally want to continue being straight. Being straight is a part of your personality after all. A strong personality trait like that could be altered if you had a stroke.
It doesn't seem all that ridiculous considering how much a stroke could affect the brain, but what I find so weird about this... is how stereotypically "gay" he is.

I don't think it's far fetched at all. I mean, being gay has to be something in the brain, right? Whether you're born with it or not shouldn't matter at all. Strokes can do all kinds of wacky things to a person, like giving them synesthesia or making them incredibly artistic, so I'm not surprised when it does something relatively benign [I](although this guy's change doesn't seem benign)[/I] like changing someone's sexual preferences.
I feel that people may object to this because usually any claim that sexuality can be changed or that homosexuality can be caused by anything, is instantly assumed to be a direct attack against all of homosexuality. It isn't.
But this case specifically seems a bit... Something.
