Have your friend think he is going to a real vasectomy (need to convince him to get one), position him so he can't see the operation, pinch his balls to simulate the needle of numbing agent, and then make a bunch smoke/burnt smells with a lighter/paper to make the guy freak out your soldering his goo veins, and then ofcourse simulate a accidental out of control fire.
Be really elaborate, but since this shit has reached "kidnap your friend and pose as ISIS" levels, might aswell try it!
