I should note, that picture looks to be from Pyongyang (seen a few documentaries).
Which means he'd be on a guided tour.
You can't get to the outside Internet from the intranet. He'd have to have a [I]satellite dish[/I], in [I]North Korea[/I], as a [I]tourist on a tourist visa[/I], after having [I]flown into the country[/I] on Air Koryo.
I suppose it would be possible to smuggle in a small-ish satellite dish from the northern border with China (where they smuggle in DVDs containing South Korean movies etc.), but at a bare minimum that's a bit over a 100 miles/160 kilometers.
This is just some guy making shitty Youtube videos.

I saw the stream. I believe he was in North Korea, except not during the livestream. He pre-recorded himself on a tour and then played it in OBS or something like that.

Well, it was fake... at least the "livestream" part anyways.
He was actually on a guided tour at some point, here's a snip of it:

He still took risk in recording that though. The guides are typically fine with you taking pictures at designated locations, but they can get upset at videos.
