Why does something that should be right look so wrong?
I'd heard of the yakuza thing, not so much the other reasons. Neat.
But sometimes it's the opposite, having fewer digits in Japan. Look at Abe the Mudokon from Oddworld.

The Mudokons have 3 fingers on each hand, but they were edited to have 4, so one could consider they still have a missing finger compared to human hands.
This whole practice seems somewhat ridiculous.

In the Oddworld reboot this seems to no longer be a thing though, and their old 4 fingered hand design is back which is cool

Because the hand was proportioned for 4 fingers. To make it look right they'd have to re-draw the entire hand, but they only re-drew the fingers.
Another reason might be the superstition that the number 4 is bad luck because it sounds like the word for death, which is pretty stupid but it's apparently a big enough deal that there's two words for the number 4, not that most people give a shit which one you use within the right contexts anyway.

Reminds me of that episode of Cobra 11 (german police drama series) where they had a case in slovakia, and there was a gag where they made fun of the language by trying to pronounce various words and failing... while voiced by native speakers in the slovak dub.
Makes me wonder how often this kind of shit happens with i.e. russian dubs of russian-speaking characters in western tv. Like, how do you have russian mafiosos talking to each other in russian without the other characters knowing what they're saying when they all speak russian?

Like how in MGS 3 someone complements big boss that his russian is great even though everything he says is in english and you realize "oh, he's actually been speaking russian this whole time"

To be fair, it's probably one of the few movies I've seen that directly addresses the "everybody speaks English" trope, without characters having to explain it. It's kinda clever.

In Black Lagoon in the Japanese audio, there's an arc where the main characters go from their south-east Asian backwater setting to Japan. The main character is Japanese and bilingual, but the other one can only speak English in-universe, so when the main characters are talking they're talking in Japanese for the convenience of the audience, but whenever she speaks to anybody else she speaks in English. It goes about as well as you would expect.

I always just treat it like the first level in Eternal Darkness, where it's clearly established that the characters are not speaking English, and then [i]movie magic[/i] makes the viewer understand them from then on.

This seems perfectly fine by me. I don't know much about that particular show, but chances are that the students are expressly japanese and it is clear that they're supposed to be speaking japanese.
I've seen it the other way around in dubs and it honestly fucks me over much more when an american character in a story that is set in america goes "whoah I can't understand this because it's not written in italian!"
