I've yet to see any game pull this off, and not have enemies just crouch walk around instead of standing

The more I think of it, the easiest way of doing it would be spawning NPC's behind the player, rather than teaching them to sneak.
which I hope is something they definitely won't do.

Don't know if it is the same thing but in Fallout 3 there was an dlc where you went in to a simulation, the chinese there used the same thing as a stealth boy and there where alot of times that i didn't actually see them so they got a jump on me.
Edit: Operation Anchorage. (had too look it up, totally forgot the name)
Maybe they could make it so sneaking enemies don't make noise, don't show on the compass, and don't affect the Hidden/Detected status?
So if you're sneaking around (or even just walking around) without looking around you, you could end up with someone tailing you without you knowing it.
I imagine that would work.
Probably wishful thinking on my part but what if some enemies could access areas the player can't and launch ambushes from there?
For example a pack of Mole Rats could clamber into a vent and come out of any of the openings the player's back is turned to. If the player doesn't feel like playing peekaboo they can counter these tactics by bringing a flamethrower or grenades to flush them out.

Half-Life kind of did this in one area.
I'm betting it's going to be more of an ambush thing like how in previous games looting certain things would trigger a group of raiders to attack.

S.T.A.L.K.E.R. does have AI sneaking around in my experience, especially in gun fights involving quite a few enemies. They mostly go by the formula of flanking and if you are not looking at their direction, hold fire and advance silently via walking until they have a good shot of your head.
I haven't really seen any real passive sneaking (no shooting, just going from A to B safely), especially since the opportunity rarely ever shows up in the wild.
I liked the way female assassins in HL1 worked, that was pretty steathy
Condemned had enemies that would try to maneuver around you, obviously not very advanced but still a basic demonstration that you can get sneaky NPCs.
Only other example would be stalker with ai mods. I think it was LURK that made them a bit better on that front.
So from what I gathered from the VATS scene, selecting enemies with lower chances to hit allows you to select more targets?
Or was that just showing that more Agility = more Action Points like in FO3?
I'm just wondering this because at first he started by selecting the enemy with the highest chance to hit, then on the second time he started with the lowest, also the "Pilling on damage to additional targets-" at the end.

That was less stealth and more them just moving really fast and jumping around a lot.
I wouldn't mind sneaky enemies that basically stay out of your sight and hide in dark spots where you can still spot them but where you might not think about looking. Kind of like how Dark Souls has a bunch of enemies perfectly visible but who you can easily ignore until it's too late.
The problem with stealthy enemies in fo3 and nv is that you could just spam the vats button and highlight people even when they had stealth boys or if you couldn't actually see them.
I'm hyped to break the game with agility.

Hopefully they change that this time around, would make enemies with stealth boys a pretty decent threat
That Mister Sandman stabbing was brutal.
(VATS doesn't pop-up unless you can actually see the enemy) would be the first step in introducing NPC-Stealth into F4
FO3's Anchorage DLC tried the stealth thing iirc. There were some dudes in the chinese stealth armor that'd try and sneak up on you with machetes/swords

That was over the top though, they backflip away from your attacks and move retardedly fast, it's not so much stealth as it is incredible agility. Stealth needs stalking behavior, not acrobatics, it'd be nice if you were legitimately unaware that you were being watched at all. They could always just make it so the sneaking enemy tries to get themselves out of your line of sight if you are looking in their direction, but stalks you if they aren't within your field of view. If they pair that with stealth boys/stealth armor then it would be a pretty potent combat behavior.
Could also do that annoyingly effective STALKER crap where the last enemy goes silent and hides in a dark corner waiting to ambush you, as well.

Pretty sure the assassins in HL1 had stupidly loud footsteps as well.

Being watched then the next time you load up the game, 15 minutes later your suddenly get stabbed/sniped/gunned down. It would make me more nervous to wander the wastes at night.

I played through that DLC very slowly and realized the Chinese Stealth Suits were just regular NPCS that stand completely still at their spawn point, but as soon as they see you they run straight to you and attack, just invisible.
What the video seems to suggest is some NPCs may see you regularly walking around and [i]then[/i] decide to sneak/follow you, or there will be dedicated sneaking NPCs that spawn in around you, like a random encounter

You couldn't shoot people using VATS if they were cloaked (0% chance to hit) but you could still activate VATS and it would zoom in on them.
