Not sure why the lady walked into the elevator without her dog right behind her in the first place. With a leash you can feel if they're right by you or not depending on how loose the leash is. Also she could've pressed the door open button or alternatively carried the dog in.

The door opened, she prevented the dog from going inside. She then stands around for a second then goes into the lift without the dog. What the fuck.

Stupid lady, but if there was any kind of metal clip or anything to tie the leash to the harness, it would catch and take all the stress instead of the pug, which seems to be what happened.
My friend is an elevator maintenance guy, and according to him this actually happens a lot. The elevator sensors can't see the leash, and it closes before the dog is in the elevator. It killed a person in my town a month ago. Someone went into the elevator while their dog was still out, then the elevator starts going up, dragging the dog with it. Someone else sees it happen and rushes to save the dog, and while she's standing against the elevator door trying to remove the leash, the door opens because the leash somehow activates a safety mechanism and the lady and the dog plunge into an open shaft. Be very, very wary around elevators when you have something on a leash or rope.
