Does he give a shit? The guy is a predator who seems to openly lust after his daughter. Kinda past "wholesome family values" with that dude.
He cares about anything that reflects badly on him. Emptying myself of any empathy, he might care that he gets sympathy votes from her illness and possible death - but that's balanced against him appearing weak for marrying 'a woman who got sick enough that she had to have surgery and (possibly) die'.
My kidneys would pop too if I had to listen to that fuckhead on a daily basis.

The point I'm making isn't that Trump definitely absolutely doesn't give a shit, but that we have 0 reason to think he DOES give a shit, and ample hints towards him only caring about himself.
Yet Trump is still a person. And judging by his behavior, he probably doesn't.
I imagine a lot of people tend to care in situations like this, but there are a few, and Trump's actions have displayed he's part of the few who pretty much dont give a flying fuck.
People can get pretty fucked up with various mental conditions. I'm not a licensed psychologist and even if I was I couldn't diagnose Trump without in-person interviews, but if he isn't suffering from Narcissistic Personality Disorder he ticks so many of the boxes it's fucking close enough to be a representative stand-in for whatever is actually fucking wrong with him.
Narcissists see other people as only holding value that can be used to benefit the narcissist. If you aren't useful to the person, what's the point of you? A narcissist's view of personal relationships is fucked up six different ways, and they are very difficult to deal with because they lie effortlessly and constantly. They will lie to you about things you have personally witnessed and have on videotape. They will lie to you about you.
While it's quite possible that, deep in his cheeseburger-clogged heart, Trump does care for and love Melania, he has a selfish and greedy personality that values people for what they can do for him. And I can imagine that, for Melania, dealing with his impulses constantly wears her down. A narcissist is no one's friend, unless they want something you have or can do. Melania, at least so far, is an attractive former model who won't divorce him (probably because she's got her eyes on the prize in his will), but it's probably very hard to love Donald because of his constant
narcissistic behaviours. There may have been love in the marriage, but by this point I doubt there's much holding them together except their wants.
