Gonna one up this with my story. If you had facebook installed on my old phone, it would add a share to facebook option when you right clicked/held down your touch on an image. I accidentally hit this option mid fap and shared some fucked up hentai right to my wall, didnt notice it until a friend messaged and told me someone must have hacked my account

One time mistook a group chat that a mentally challenged (like severely autistic or some sort of mental retardation) co-worker of mine had added me to for his birthday party as a meme chat some of my friends are in. I called someone who left the birthday chat a "cuck" because I believed it was someone leaving our meme chat. The mentally challenged co-worker then asked "what is a cuck?" which was when I realized my mistake.

I accidentally sent erotica I was writing to the wrong person on my steam friends list :^)

Usually posting porn in the wrong channel, or pinging everyone when absolutely newscasted.
