need help with my report/ticket system

Let me explain:
When a user click "send ticket, the info (option and description) should be saved in a button. The button will automatically create itself. On the button is only the name of the player and the argument. Like in this Screenshot:

If you click the button the info(argument and description) will show up on the right side.
Can you guys help me out with that?
