Extravagant anime models are cool and all, but I think I like these kinds of creative, abstract-looking (and more performant) avatars more.
I was surprised to find that VRChat actually runs smoothly with 30+ people, as long as everyone has optimized avatars. My computer can barely handle 5-6 MMD models in a public server, makes me dislike anime people quite a bit.

What's the best way to get more performance out of the game? I'm on a 4690k with 16 gigs of ram and a 1060 and sometimes I hit 12 fps. It's awful, man.
Absolutely best way would be to hang out with people that have more optimized avatars in more optimized worlds, a lot of world creators are desktop only users and don't realize that even a couple of realtime lights could ruin the framerate of any VR user in the room. Ofcourse upgrading your rig would help, but even with my i7 6700k and 1080ti I'll get destroyed by a couple of bad avatars in a badly optimized world.
SteamVR is buggy garbage if you're using a Rift. It will sometimes drop the FPS to 11 for no good reason, and the only way to fix it is to restart both SteamVR and Oculus Home.
Its mostly because those people using the MMD models don't bother to clean them up and use edited SDKs to void the tri cap, so they're running around with models in excess of a million tris with fully modelled body internals because they never bothered to check if the model was built upon a 18+ base, and then theres the massive amount of smaller bits of mesh for all the facial flexes that they never bother to actually setup so they're sat in the model's head, scrunched all the way down to the degree they're z-fighting like fuck which again stresses everyones GPU. The fact that people don't bother to cleanup and optimise their mesh is the fault here.
And then you get the ones that optimize the model but don't do fuck all with the materials. Like cmon ya fuck it takes not time to texture atlas.
Can you plop the model with materials back into Blender from Unity for Atlasing? Since I've set up my materials in Unity and not in Blender.
Maybe, you can try an FBX exporter (it probably won't export with textures, or texture offsets if you applied any). I believe it'd be less trouble to just atlas it and re-texture.
There's some bizarre bug in VRChat that somehow makes the game use way more of your GPU than it should be under some circumstances. I have a i7-4790K with a 970, and I run into this issue all the time. I've been told it might have something to do with the game not clearing the GPU's cache. There are several things that can trigger the bug. The most common is simply loading into a graphically intense world, especially one where there's a mirror that's on by default. The second is loading into a world with a lot of people that are within your view frustum when you spawn. The third is opening the steamVR menu in front of a mirror or a bunch of people. There is a fix if you're on a vive. Opening the steamVR menu while looking away from anything graphically intensive (make sure mirrors are completely off) and opening and closing the steamVR menu will fix the issue 30% of the time. If that fails, the method I've found that works 100% of the time is to go to the steamVR settings, find the video tab, check manual override, and drop your resolution until the frame rate bar in the lower left stops showing dropped frames in red. Then increase the resolution by 2-10%. You''ll see a bunch of dropped frames. Open and close the steamVR menu and there should no longer be any dropped frames. If there are, drop the resolution again and increase in a smaller step. Repeat the process until you return to 100% resolution. You should be at a much higher frame-rate by the end. I think this won't work on Occulus, as it doesn't use steamVR's super-sampling settings. This issue is obviously a bug and not a result of weak hardware, as I can go from sub 11-fps to 90 in some cases by doing this. I'm fairly certain it's not steamVR's fault, as no other VR game I've tried has the issue. The problem almost certainly lies in the ancient unity version VRChat is using, or something stupid the game itself is doing.

Are there guides out there for making an avatar from scratch? Preferably for someone who hasn't done any 3d modeling before.
I don't think you'll find anything comprehensive from scratch to VRChat, but if you split the tasks up into chunks it becomes a bit easier to find content on it. This is how I would proceed: If I had a tablet and valued traditional art, investigate ZBrush workflows. A ZBrush workflow would look like this: ZBrush sculpt->Blender retopology->Blender uv unwrap->Substance texturing/baking->Blender rigging->Blender visemes->Unity setup->VRChat SDK upload. Otherwise I would investigate cube/subdivision modeling, a long standing classic approach to modelling which doesn't require any expensive technology or a steady hand. A typical cube modelling workflow would look like this: Blender cube model->Blender uv unwrap->Substance texturing/baking->Blender rigging->Blender visemes->Unity setup->VRChat SDK upload. There's only a few differences between the workflows, I personally use cube modelling because you can skip retopology since subdivision modelling comes with built-in LOD. I kinda detailed out the workflow so that you can figure out each part like "How to ZBrush sculpt a character", or "How to blender uv unwrap" will bring up some pretty straightforward tutorials.

I just got myself a Vive last night since I got paid the other day, and decided to treat myself. I'm finding people in VRchat really quiet, and it seems it might be all games are quite quiet. I'm plugging in a headset into the jack that's provided, but I have a feeling it's not powerful enough to give me the sound I need. Should I be realistically investing in some wireless headphones, or would anyone have a suggestion they'd be willing to share?
For me the headsets that came with the Vive on the beginning we're perfect, not too quiet and not too loud. Are you sure it's not that you have your volume lowered on your PC?
The earbuds that came with the Vive? I have the regular Vive, not the pro. The earbuds don't stay in my ears and don't really supply enough oomf. I looked at all my settings and everything was up 100%. Also, someone suggested unchecking a setting for levels in the Vive and I did that with no change.
