well then you add your icon into your addon as a material and do the exact same thing as listed in the wiki? Im not going to make that for you, if you can't code that im seriously wondering what the hell you're making anyways.
what file should I put it in?

Maybe just maybe, you put your materials in a folder in the materials folder.
