I thought this was gonna be a total over reaction to the increased skin cancer risk from thiazide, but this is obviously a bit different. How does this happen more than once?
Ah, so it's not the drug itself, but a contaminant in the drug. Those are quite different from each other.
How does this happen? They say it's the result of the manufacturing process, that's not very descriptive.
I'd guess during manufacturing. Some of the most complex drugs might have dozens of chemical steps with dozens of different things reacted to them during the process. All it takes is one to be contaminated with something (and it doesn't ahve to be this cancer causing chemical) and you get a funny side reaction that makes the contaminant.
It was the change in manufacturing process that caused the issue. The new process resulted in the production of an unanticipated impurity, so it was not tested for until recently.

Still strange, they should probably be required to share the cause of the issue, not for the public, but so other manufacturers don't make the same error. If this wasn't found sooner the damage may have been pretty severe.
Yeah, even if they didn't disclose anything (somewhat understandable as these are considered trade secrets), I'd be looking at all other manufacturers of similar drugs and reviewing their products as well as processes.
