There seems to be a bug when you have a full inventory AND all clothes slots filled. Die and allow your player to disappear which leaves the backback. One of your items will be gone.
Steps to reproduce:
Fill inventory with items
Also fill all clothing slots
suicide
wait for body to despawn and backpack loot box to spawn
You've now lost an item... congrats
Notes:
Bag doesn't seem to despawn
Looting all "available" items (excluding the missing item of course) does not cause the bag to despawn
I assume this is caused by the fact that you normally have 43 slots including clothing slots. The backpack only has 42 and the game doesn't know what to do with the extra item.
This happened to me after raiding someone. Just got a bolt with all attachments and was very happy with myself. Went back to my cave base and did the suicide drop. Spawned near the raid target to get the rest of the loot. When i finally got back the bolt wasnt in the inventory. Everything else was.
Fix:
Add an extra slot to the backpack loot container
