My best guess was that he didn't wait for his nose to fully heal after his plastic surgery and continued the cocaine abuse, thereby breaking down the tissue that was slowly growing back, and allowing cartilage to collapse. One good smack on the nose or even sleeping with pressure on it could end up with that result.
