Its a common misconception that U.S. prisons detain criminals (Y'know when they actually detain real threats to society) when in actuality the states give them what is essentially a well fortified castle that has a captive market and constant new blood to indoctrinate. Prisoners may be stupid enough to get into prison but they are smart enough to organize themselves.
Guards sometimes even have their families threatened if they try to interfere with inter-prisoner violence and it'd be a shame if they caught some glass to the jugular by accident one day.
Make no mistake murders like these are premeditated, calculated and mean't to send a message. The guards were most likely told to fuck off and look the other way.

Erm, have you ever gone hunting? Gutted a fish? Carved a turkey? Prepped a chicken for baking? Any of that would tell you that removing limbs from a human would be extremely difficult, especially with handmade prison tools. This is common sense man.
