I am currently developing a stalker misery roleplay, and of course, i am using NPCs for mutants, and I am facing a very weird issue, that some one you might have the solution.
So i discovered that : (Tested with a mutant 150 HP, 50 damage per hit, checked with logs)
On a small map : We can have any mutant that we want and any player number that we want, all bullet hit will count, hitboxes are right, no problem.
On a big map :
-If i am singleplayer, or multiplayer with no other players, hitbox are also fine, mutants die in 3 hits, it's okay.
-If multiplayer, and with many others players connected in the same location, same render zone/distance, hitboxe still are fine, mutants die in 3 hits, it's okay.
-If multiplayer, and with many others players connected BUT, very distant, not rendering the other player, the hitboxes are fucked up and i need more than 15 hit to get down that NPC, the hitbox fail scales down with the player count.
Tried with both custom NPC, and vanila gmod NPCs, and Custom weapons and vanila weapons.
Do you guys have any idea of what's going wrong ?

I had the same error, i just ended up giving up to the map, going back to a shitty one at the cost of my playerbase, this is really a huge issue to servers that attempt to create an original gameplay, i hope someone have a solution

Gmod is not a game for that big roleplay.
I'd recommend you change development platform.
