Well he just came back from a pretty serious heart surgery a couple of months ago which haulted all of his workouts. He's back at it now but still could of caused issues

I mean it's definitely not surprising that it wasn't a serious injury; I'm not saying that if you get kicked in the back you may as well start thinking about wheelchair colors.
My point is really just that humans are far more fragile than we like to think, even huge guys like Arnie. And that we tend to forget that extremely serious consequences can result from seemingly harmless actions.
That dude wasn't trying to paralyze or kill Arnold, but if things went exactly wrong that's what he would have done. And that's fucking terrifying as a human being. To me anyways
Definitely this, but I'll mention this - once you've used steroids there are permanent changes to your body. Gaining muscle is significantly easier due to myo-nuclear adaptations. Odds are even if he were relatively sedentary he would carry more muscle mass than most of us.
That's good to see, because years back, he'd really let himself go.
