Face punch will eventually make this happen i reckon but there should be rust players known as Psychos, the so called "Psychos" are created by normal player killing a lot of players making their back hunch and have a more sadistic facial expression also the should have either cracked or rough skin and they should get very pale (this would also happen to cannibals). cannibals would always have blood on or near their mouth area and a cannibal's get hungry quicker and they get damaged if they eat anything but human flesh. the effects of being a psycho starts when player kill over 60 players in a day. to be a cannibal you must eat human flesh 20 times before turning into one. these effect wear off after a wipe or if you are on a different server.
When a person kills 60 people a day, then he usually wears a facemask or a helmet > No face expression can be seen xd
well it doesn't matter too much its mostly the skin posture and thirst to kill is pretty east to see
